This form is to be used by Attorneys, Trustees, Creditors, and other authorized parties who are seeking filing privileges to this Court's Case Management / Electronic Case Filing System (CM/ECF). Filing access is assigned based on the applicant type. Refer to Local Rule 1001-2 (Case Management and Electronic Case Filing System – CM/ECF) for the Court's CM/ECF practices and Local Rule 2090-1 (Admission to Practice) for information on the Court’s admission requirements. Completion of this registration form, in addition to registering through PACER, is required. The information will be used only for that purpose and will be kept confidential. Additional filing privileges may be added at the discretion of the Clerk.

  By submitting this registration form, I agree to abide by the following rules:  
     
  1. Pursuant to Federal Rule of Bankruptcy Procedure 9011, every pleading, motion and other paper (except lists, schedules, statements or amendments thereto) filed with the court shall be signed by at least one attorney of record or by the party filing the pleading if not represented by an attorney. The unique password issued to a user registered to use the Court's CM/ECF system identifies that user to the court each time the user logs into CM/ECF. The use of a user's password serves as and constitutes the signature of the user for purposes of Bankruptcy Rule 9011 on any document or pleading filed electronically.  
     
  2. The user must protect and secure the password issued by the court. If any reason exists to suspect the password has been compromised in any way, it is the duty and responsibility of the user to notify the Court's CM/ECF HelpDesk, ecfhelp@flmb.usourts.gov, immediately. The clerk will immediately disable filing access until that password has been reset through PACER.  
     
  3. Certification to file in the electronic case filing system will constitute a request and an agreement to receive service of pleadings and other papers electronically pursuant to Federal Rule of Bankruptcy Procedure 9036, where service of pleadings is otherwise permitted by first class mail, postage prepaid.

Approvals and rejections of filing access to the Court’s CM/ECF System are received directly from PACER, do_not_reply@psc.uscourts.gov.
